TERATOMA KISTIK MATUR DENGAN TRANSFORMASI KEGANASAN TIPE KARSINOMA PAPILER TIROID VARIAN FOLIKULAR PADA OVARIUM KANAN

                   Teratoma kistik matur merupakan tumor ovarium yang paling sering terjadi pada wanita usia reproduktif. Transformasi keganasan pada teratoma kistik matur jarang terjadi. Transformasi keganasan yang paling umum pada teratoma kistik matur adalah karsinoma sel skuamosa, sedangkan karsinoma tiroid papiler merupakan kejadian yang sangat langka. Kami melaporkan satu kasus  Karsinoma Papiler Tiroid Varian Folikular yang timbul dari teratoma kistik matur di ovarium kanan, pada wanita berusia 34 tahun dengan teratoma kistik matur bilateral. Pasien menjalani tindakan salpingo-ooforektomi bilateral. Pemeriksaan mikroskopis ovarium kanan dan kiri menunjukkan gambaran kista dilapisi epitel skuamosa kompleks dengan adneksa kulit berupa folikel rambut dan kelenjar sebacea serta dijumpai tulang rawan matur. Pada ovarium kanan ditemukan komponen jaringan tiroid, membentuk struktur folikular padat, sedikit struktur papiler, dilapisi epitel kuboid hingga kolumner, selapis hingga pseudostratifikasi, inti Sebagian overlapping, bulat-ovoid, pleomorfik, gambaran inti menunjukkan nuclear groove, nuclear clearing, Orphan Annie eyes, dan nuclear pseudo-inclusi, sitoplasma eosinofilik. Jumlah mitosis sedikit. Pengecatan IHK TTF-1 menunjukkan hasil positif. Ahli patologi harus melaporkan transformasi keganasan pada teratoma kistik matur agar klinisi dapat memberikan terapi yang spesifik dan adekuat pada pasien.

Abstract

Mature cystic teratomas are the most common ovarian germ cell tumors in women of reproductive age. The malignant transformation of mature cystic teratomas is a rare entity and the most common malignant transformation is squamous cell carcinoma, whereas papillary thyroid carcinoma is an exceptional event. We reported  a case of Follicular Variant of  Papillary Thyroid Carcinoma arising from mature cystic teratoma in the right ovary, in 34 year old woman with  bilateral mature cystic teratomas. The patient underwent bilateral salpingo-oophorectomy. Microscopic examination of bilateral ovarii showed cyst lined by stratified squamous epithelium with mature skin appendages (hair follicles and sebaceous glands) and mature cartilage component. In the right ovary we found a thyroid tissue with well-developed follicular, solid and  focal papillary structures, composed of cuboidal to columner epithelial. The nuclei have crowded and overlapping, round to ovoid, pleomorphic, with ground glass (‘Orphan Annie’ eye) appearance, pseudo-inclusions and nuclear grooves with eosinophilic cytoplasm. The mitotic figures were rare. The result of TTF-1 immunostaining was positive. Pathologists should be aware of malignant transformation and exclude it when investigating mature cystic teratomas in order to specific and adequate therapy.
